Ive been reading about 80’s teen sex comedies and Cthulhu and Freud stuff
So it got me thinking that the mechanics of horror and sanity loss are quite similar to the pacing of American pie type stuff, Call of Tiddies if you will
PC’s are horny students, dirty old folks and so on, they got a boner/lust meter that causes involuntary actions. Perhaps the goal is to fill the meter but not get your sex organ grabbed by a cranky gym teacher

As someone who never watched Porky's...
Is the general gist Cybersex with Tabletop Mechanics, or is it primarily a comedy game with sexual elements? While teen sex comedies are not by nature pornographic, the nature of the tabletop fandom means that people may view your game as a means of acting out their fantasies.
If that's what you want and advertise, that's cool. But it's not then you need to make this clear.

The problem with the sanity meter is that it's not dynamic, it's just hit points for the adventure. In a typical teenage sex comedy, you have titillation contrasted with boner killing for nominally comedic effect. Which would represent going up on the scale and which would represent going down? Either way, it doesn't work very well with the sanity scale system because the sanity meter only goes one direction until the end of the adventure.
So you see the cheerleaders doing a car wash. Then a kindly grandma walks into the frame and starts asking for directions. The comedy is that the characters have to explain to an old lady how to get to the library while hiding that they were getting erections watching cheerleaders bend over the hoods of cars. But which of these events is supposed to be represented by the meter going down? They are directly sequential contrasting events, that's the structure of the comedy.

Yeah the boner meter needs to be filled and decreased with different actions. Perhaps something like the stress system mentioned in Assymetric Threat. Certain character building and action choices can give you permanent horny points

Or the penalty for getting too high on the boner meter is that the universe finds some comedic un-sexy thing to happen to lower it? The un-sexy thing also distracts the players, because they're required to deal with it before they can proceed. During which time they have lost whatever opportunity they were going for in that scene?
Although you'd need hard and fast (heh) rules about what happens when, or it's just GM bullshit ruining your game. It already sounds like Paradox in Mage, which isn't a great sign.

Say in the shining, the father's boner meter gets raised when he sees the bathing woman, but then it's smashed to bits when the embrace of the water nymph is that of a rotting dead old person cackling.
As the goal is humor for this game some bullshit-yet-funny random encounter/fumble/critical tables with different themes could work. It can also be a pervert shonen manga concept generator like you start off with your horny student characters but then roll off to see if the big sexy will come in the form of a space invaders, sexy demons, super science gynoids, etc.
